Medical Box, 410-1, 583, first stroke, 3641 Woodstdale Road, Swinghaven Avenue and Walden Road. Operation Zon Amats 107. This is not. Clear. We're after the facility on board of the 71-year-old female alert in order for GCS 15[1]. Chief complaint with abdominal pain.

Patient was discharged from your facility approximately seven hours ago with, I believe it was diperticulosis, not acute diabetes. Paciaseaseous has been an ongoing issue for quite a while. I did have rectal bleeding. She states that center controls. Her biggest complaint right now is abdominal pain as well as some nausea.

She did state on her initial arrival that she had some numbness of her right hand and her frontal headache. She was stating she had some confusion, couldn't think straight. However, she's okay now. She's stating if this all started around 12 o'clock tonight, known well, it would have been 1150. Lambs were negative, Cincinnati was negative.

She did have some hypertension. Last pressure was 202 over 94. Most recent pressure now is 184 over 77. She does not take anything for blood pressure. Three leans show normal sinus further motorometer, 98% remare. Glucose of 212. We got with 20 games in her rate AC. She did get 8 milligrams per diaphragm and IV.
