On team with an 86, 86-year-old male. Chief Complaint of Weakness, possible CVA starting yesterday at 1630 hours. According to the White, patient was acting normal up until they arrived home after driving at about 1630 hours yesterday when they noticed he was getting bilaterally weak, having trouble walking and standing. They reported that he had a near-thinkable episode but did not actually pass out. Patient had did not fall. He's been awake alert and orientate the entire time for me.

This morning, a patient was found to have increasing weakness. They called their physician who stated that he's worried that the patient may have had an acute TIA. Patient did have a central line placed yesterday at an outpatient facility. Right now, my stroke assessment on the Lambs and Cincinnati scales are both negative. However, he does have left leg drift.

and an abnormal gait, he is complaining of a headache to the anterior portion on the right side of his head. He reports no numbness, no tangling, no blurred vision, no double vision. However, with his presentation, with the headache, the abnormal gait and the weakness, and the left leg drift, I am concerned that he may have had a CBA or TIA at this point. Vital's are as follows. Pulse 65, sinus rhythm on the monitor. I sent you my 12th lead, which is unremarkable.

97% on room air. Blood pressures 168 over 90 blood sugar 242. He's breathing 16 times a minute. Again, negative on the Cincinnati and Lamb scales. However, I am going to call stroke alert just on this presentation with the left leg drift, weakness, abnormal gait, as well as the headache. I am approximately one and a half minutes away from you on literally across the street.
