Subject missing from the care facility at (number withheld) Van Nuys Boulevard.

Subject (name withheld), a male, black, brown, brown, five nine, 190 , date of birth (number withheld) , (number withheld) , wearing a white shirt, blue pants.

Subject is diagnosed with Alzheimer's and dementia.

Refused to return to the facility and boarded an MTA bus, last seen northbound on Van Nuys Boulevard from Vanowen, at approximately 1230 hours.
