Patient had a mechanical ground level fall, did hit his head, did not lose consciousness, was witnessed.

Is not on any blood thinners and I am seeing current set of vitals got a heart rate of seventy-two, we're at ninety-four percent room air, got a blood pressure one thirty-four over fifty-four.

He is a DNR and he is on hospice.
