Woman with declining health after multiple falls at care center

Unit at the Rib Lake Health Center, (number withheld) Pearl Street, for a 59-year-old female resident with just an overall declining health.

She's fallen a couple times in the past few days and is talking, not making much sense.

So they want her taken out.
