Person found in cardiac arrest dies after resuscitation
Heard on:
Carver County MN Regional EMHRV_PI (Hospital)
04:01
00:00
Ridgeview, 803, looking for a doc on a cardiac arrest.
00:04
Patient was pulled on by finding her.
00:10
She started CPR.
00:12
Fire got here shortly after.
00:14
Took over. We do have her on the, have him on the Lucas currently.
00:19
Patient is intubated.
00:21
Our current end tidal is 61.
00:26
We do have an IO in place.
00:28
We have given a total of three epi, one bicarb, and one.
00:33
Initial
00:34
blood sugar was thirty-three.
00:38
History of COPD,
00:42
AFIB, and high blood pressure.
00:44
We have currently been on here for about a half an hour with no change.
00:50
Patient has remained AFIB solid for us.
00:54
Just wondering if you have any other thoughts that we could try at this point.
01:01
So it sounds like you've done everything correctly.
01:04
I'm just confirming that the patient has been always asystolic and never any shockable rhythm.
01:10
Is that correct?
01:12
Or that is correct.
01:14
We did have one or two random ectopic beats at pulse checks, but nothing sustained.
01:22
Yeah, I would recommend trying to give just either 1 g of calcium chloride or three of calcium gluconate, whichever you have available.
01:33
And then I would do one more round of ACLS, including another of epinephrine.
01:38
But given that this patient unfortunately was found down the last known well last night, has been asystolic for this time period, intubation demonstrates.
01:48
It sounds like they got intubated.
01:50
I perceive that this is unfortunately futile here after that point, and would advise terminating resuscitative efforts.
02:03
Unless you have any other thoughts or considerations, or family is very hesitant, we're happy to see them here and transfer.
02:10
But it sounds like you've done everything correctly.
02:13
But yeah, I would recommend trying that calcium administration, and one more round with.
02:19
Epi before deciding to terminate.
02:23
Copy.
02:25
Just repeating back, we are going to do another.
02:31
We're going to do another round of ACLS here, including epi, as well as a gram of potassium chloride.
02:39
We'll give you a call in a few minutes and let you know if we had any changes.
02:46
I'll wait to hear.
02:50
Radio 803 to med control.
02:55
After landing, go ahead.
02:59
So we have given the calcium chloride 1 g as well as an additional heparin, and we've been doing CPR for about 2 min here.
03:12
Currently, the only change we have, unfortunately, is.
03:16
The pen title for the PT tube is now full of blood, and we are consistently suctioning.
03:25
No change in rhythm.
03:29
We are still looking at in phase as well.
03:32
At this point, we are looking for orders to terminate the sedation effort.
03:42
Copy that. Yes, it does sound like all appropriate resuscitative measures have been provided, and any further care would be futile.
03:50
And at this time, I would advise terminating resuscitative efforts and stopping CPR.
03:59
Time of death at 6:23.
